Note: In addition to filling out this form, the following is also required:

A non-refundable $35.00 application fee (can be paid by calling the cashier's office w/ credit card info at (435) 652-7605, or mail to: Dixie State College Admissions Office, 225 S. 700 E. St. George, UT. 84770. If you have attended DSC previously, then instead pay a $10.00 readmission fee.
Copy of high school transcript (contact high school counselor, and have copy sent to same address as above).
Copy of ACT or SAT scores if not already on the high school transcript. CPT test may be taken in lieu of the ACT or SAT. Contact the Testing Center at (435) 652-7692 for additional information.
For Transfer Students, send official transcripts from all previously attended colleges/universities. If fewer than 25 credits are transferring, also need to send a high school transcript and take a placement test (ACT, SAT, CPT).
