Procedure for Reserving a Dixie State College
Motor Pool Car:
It’s as easy as 1,2,3
1. Obtain required Travel Authorization slip.
2. Fill out the TA form with authorized signatures.
3. Phone Felica Griswold at Extension #7566. Have the following information available:
a.
Name of Authorized Driver
b.
Type of vehicle required (ie. compact, sedan, 7 passenger mini van, 15
passenger van)
c.
Travel Authorization number
d.
Department account number for charging information
e.
Date of departure and return
f.
Destination

There
will be a ‘packet’ issued to each vehicle which will contain insurance
information, roadside assistance information and gas credit cards.
The State Gas card will be the primary card used for fueling during
your travel.
